Understanding & Using Your Plan

We focus on coordinating your network of supports, planning for your future needs, and support you with anything unexpected along the way. This may include:

  • Understanding your NDIS plan, with a focus on creativity, flexibility and value for money.

  • Working with you to set up service agreements with your NDIS providers.

  • Making sure your disability supports are working well for you and resolving any issues.

  • Connecting you with other providers such as early learning services or schools, health providers, government departments, accommodation services and employers.

  • Developing your informal or community connections to enhance your support network.

  • Building your skills and confidence, so you can coordinate your supports in future.
     

Some times our participants have everything they need to succeed, and their current supports are already working - they just require the resources and support from someone to bounce ideas off and check they are on the right track - We can help!

We've got years of experience with supporting individuals with a range of hurdles within the NDIS, we can play a big or small role in supporting you to manage your plan and make any changes that may be necessary, by creating an individual support plan that works best for you and your circumstances and adjusting as required.

When it comes to navigating and coordinating your disability supports, the most important person in every NDIS plan is you. That’s why we work with you, one-to-one, to support you to coordinate your supports and manage your funding in order to achieve your NDIS goals.

Our teams knowledge and lived experience helps us to really understand what your daily activities look like and work with you to create a support system that perfectly fits your individual needs.

This collaboration is essential for capacity building and empowering our clients to have their say and full choice and control in using their NDIS plan.

Developing your Support Team

We have an indepth understanding of the challenges experienced in accessing supports in Regional WA.

We will work with you to understand your goals, individual needs, and whats most important to you when creating your support team.

Your supports may consist of a range of modes including face to face supports, telehealth supports, intensive supports, and/or education and training.

Support Coordination: Projects

Specialist Support Coordination

Specialist Support Coordination may be funded in your NDIS plan under Capacity Building, Level 3 - Specialist Support Coordiantion, and may be included in your plan where additional or intensive support is required.

In addition to what your support coordinator would generally be expected to do, your specialist support coordinator can help you:

  • address complex barriers that affect your ability to access the right supports

  • design a service plan for your specific support needs, where needed.

Your specialist support coordinator should work with you, your family and carers to:

  • understand what might impact your ability to access and maintain services and supports

  • find ways to overcome these challenges

  • set up a process to overcome these barriers.

  • makes sure everyone can work together to help you pursue your goals

  • explains how everyone should communicate and cooperate with each other to support you

  • helps you, your family and carer to manage problems or unexpected situations

  • makes sure you keep getting the services you need in a crisis.

Our years of experience as users and workers in the industry have allowed us to have an in depth understanding of how the NDIS operates and how to handle the changes. 


Our team of experts is ready to help you develop strategies to not only understand your role as a Support Coordinator, but equip you to adapt to the changes, now and in the future.

We are also available for weekly, fortnightly and/or monthly peer supervision sessions to provide a sounding board for those trickier cases or simply help guide you towards positive outcomes for your participants.
